I Jason Culver uh hereby voluntarily consent to allow officer zielinski a member of the South Plainfield Police Department and any other representative designated to assist take blood samples from me and I've voluntarily consent to the testing of my blood samples I have been advised by officer zielinski and fully understand that I have the right to refuse giving my consent to the taking and testing of blood samples I have been further advised that I may withdraw my consent at any time and for any reason up until the commencement of the taking of my blood sample site I have knowingly and voluntarily given my written consent to the taking and testing of blood samples uh is that interested in science I'm going to say negative okay so then I'm going to have to call up the County Department the reason I'm calling you is uh I had a two-car rollover motor vehicle accident we have a suspicion that the driver was operating on under the influence of alcohol he was arrested and we were at the hospital now and we're seeking a blood draw warrant he did not consent for wardrobe I'm sorry what why can't he he denied consent yeah we're at the hospital so the alcohol test is out the window so there was Witnesses on scene that observed the motor vehicle accident they helped him out of the vehicle we responded to the scene shortly after um he smelled a strong alcoholic beverage I mean from his breath his eyes were bloodshot and watery Silas are droopy he was swaying as he was standing he was swaying as he walked I performed hgn Oregon horizontal gaze nystagmus I observed lack of smooth pursuit in the left and right eye uh well so the odor um the poorly performed sfsts yeah okay I think that's all I got for you watch out watery eyes and then yeah you know it's a Jason Culver he's uh police department no so the other driver the other vehicle is just the parked vehicle so we hit a parked vehicle and his vehicle overturned on the side um he wasn't injured he uh denied EMS services on scene we got back into the booking room during the booking process he stated that he had 80s and he wished to be transported glass not trying to give me a long time I mean unfortunately on the warrant itself it says by any reasonable Force Justified or whatever I'll redo the warmer but no I really I really don't want to go that route I didn't want to go this route but uh yeah if you just make you know one phone call just get somebody to come pick you up I have the the form as soon as somebody comes here after the blood drawn signs the form uh you know I'll be on my way no I can have somebody pick me up here or something yeah all I got to do is uh sign the potential liability form as soon as you know I'll hopefully be getting the blood draw within the next 10 minutes 15 minutes so really the the sooner somebody comes and signs the sooner I'll be out of here please make sure they're sober yeah all right you want to pick me up with JFK I don't want to put it
