session of wearing the man bra hello are you okay i'm all turned up oh no i'm not no i'm not apparently it's going to be a fish oily lens on us i'm going to make your face look even fatter than it is where's the button jamie it's there we'll reveal what it's really like so where are you hurting to be a paramedic hello there lawrence we're with the west midlands ambulance service we get life to get death we have a bit of everything as they deal with three 000 emergency calls each day we're not in the bronx yes taking you right to the heart of the action you can't all right that's the worst my lips are so short it's unbelievable [Music] right where we going willing not rustling your lips are painting now girl my lips are pounding are they really because they're so sore i don't know leave it on i just got that lovely shine off if it looks like you don't please don't you're not pissing me no i never said i was going through that bitty boot yeah but i just don't put it on my lids i put it all over here as well because i know we've noticed right up to your nose your lips look about an inch i once went to a job where the family we dealt with the patient um we'd done our assessment and we were about to take them to the hospital and the family said um we just about to have dinner can we interest you in some dinner and i was like no honestly i'm fine thank you no no you must you must have something they literally laid out a three-course meal for us at the time we didn't stay obviously however they very kindly did do us a little doggy bag so he literally kept us going the whole ship michelle mcnulty and hannah simkins have just come off a break and within seconds they get a call what we got [Music] so we're 16.5 to a red back up for a gentleman in his 40s clear left who is suspected of having a stroke in the paradigm who keeps collapsing and he's got speech difficulties nice skills skills what's going on today mate a rapid responder is already on the scene with the patient who's extremely distressed [Music] he works in a warehouse packing car parts but has been off sick for six months there you go cog control can't control what can't you control there's nothing to be frightened of okay okay we're here [Music] six months ago paul was diagnosed with severe anxiety and depression he desperately wants to talk to his wife of 23 years mandy shall we try and do it again you can't read no credit you've got no credit we can wrinkle don't worry okay paul's been on anti-anxiety medication since his diagnosis you say it was your first day back at work today yeah yes yesterday yeah and what did she say he said he would get ease that would today it didn't feel easier people wouldn't talk to me you feel sick you're not silly we're seeing more and more people like paul in his situation every day every shift that we come in it just seems to be on the increase there's still a massive stigma attached to it in society so they try and cope with it for as long as they can and then get the point where paula got to which is like a sort of crisis okay let's just take some deep calm breaths in that's it [Music] square breathing [Music] and what about your gp is he aware of everything and he knows that he went back to work yesterday so was it his recommendation that you went [Music] to detroit i think maybe stop worrying about that for the minute and at least you know you tried didn't you it's a very brave thing to do last three jobs before this one i was physically bullied mentally bullied [Music] okay yeah is it monday do you want to get mandy's number off and then we'll try and give her a call paul met mandy 33 years ago at a country in western night there's nothing else on there just three numbers monday mobile you're 57. with paul's breathing slowing down michelle wants to examine him what's happening to your arm here you scratched it how did you do that i was taking the printer apart just horrible thoughts that go through weird bonnie slipper really don't worry don't worry okay is it sorry can i just take this finger pull is it is it more that you're sad or that you're angry or absolutely losing everything sharp scratch well done when you say that you're frightened of losing everything obviously i know that you've already said your job is there anything else that you're frightened of losing my name's hannah i'm a paramedic with the ambulance service is that mandy we've um we've we're with paul at the moment he's left work because he's not feeling particularly well so we're going to pop him up to pop him up to the hospital but obviously he wants uh wants somebody there [Music] michelle mcnulty and hannah simkins are helping paul who suffers with severe anxiety he desperately wants his wife but a phone call to her got cut off let's try and let's try and control this breathing again look at me slow down slowly deeply deep i know [Music] hello hello is that monday i just want you to know we're with paul um he's obviously told us about what's been going on in the way that he's been um feeling recently okay um what we're gonna do is take him to the hospital and we're gonna take him to warsaw manor but we're wondering if you could um come up to the hospital to meet him because he obviously wants to have someone to talk to okay okay all right that's brilliant thank you very much bye bye she's on her way paul all right she's going to meet us at the hospital [Music] after spending 40 minutes trying to calm paul down they prepare to head to hospital [Music] let her down you haven't let it down before you tried your best it's all open and you still can oh you know how brave it is for you to admit hey and look for help and want help yeah because you want to be there for monday and be daughter don't you you haven't let anybody down you really haven't you just need to remember there's people here to help you all right come on look at me hey no one's saying that you've got to stay [Music] say try and keep yourself calm okay do that breathing again with the square let's get the square let's do the breathing again okay that's it that's it well done don't be nervous don't be nervous on then bring that with you because that's really effective mandy's on her way up all right if you can stand up for me come on i don't know you're not on your own i'm with you i'm with you come that's it [Music] michelle and hannah are straight on to their next emergency call but paul is still on their mind i find it difficult with those so you don't always know what to say so difficult having not been through something like that you know to me i feel quite exhausted actually yeah it's like really mentally draining isn't it like trying to just keep them talking and try and take their mind off it and get them to just focus i mean keep getting him try and focus on the breathing and stuff that's a really good technique though i don't think i've ever seen that before the square with the counting i've heard of it yeah smashed it dude [Music] what is the most you've ever eaten achieved the most i've eaten on one shift was christmas day just gone i did a nine nine so i went to my mum's before work and she did breakfast because we were working christmas day we were gonna have a good time we made the truck into a reindeer we've got reindeer antlers and a red nose on the truck we've got tinsel everywhere and we i bought like a christmas picnic we've got enough food to feed an army we've got sandwiches sausage rolls all your little party food crisps christmas cake we've got tins of chocolate literally we did not stop eating it's when you have to loosen your work belt that's when you know you've eaten too much in a shift [Music] i've got that stuck in my head now what casualty themed that programme makes me laugh dude i don't watch it anymore you should just sit there shout and see my wife what is it really did you say every week there was one episode where they went to a train crash a massive rtc and a guide falling for a roof and i'm like i can't even get one of them in six years yeah do you know they all go to the pub at the end of shift and have a nice drink because they're not fun i mean yeah tired after their 12-hour shift [Music] do you watch medical programmes when you're at home i do like some of them it just depends her real life it is i like 24 hours in a e i love that because that's that's very like real life isn't it it's like it depends what mood i'm in if i've been at work and i've had a long day the last thing i want to do is switch on a programme about anything remotely medical it's monday afternoon and jamie cashmore and carly manning are responding to a 999 call from a doctor's surgery cctv in operation i feel like tending we've got our room a patient has pains in his chest and is struggling to breathe hey there hello sir right my name is jeremy right what was your what was your name sorry carl carl is 41 and works in a warehouse loading lorries he's been having chest pains on and off for a few months but today at work they got worse and it became difficult to breathe his boss sent him straight to the gp you know when you're running yeah and you feel it and i can't catch the rest of you and when i was at work i felt who's going to pass out at one point i feel like i'm trying to force a celtic i can't get enough air yeah like yeah by my heart i thought you know yeah he's always panicking yourself that's one thinking is thought he was having a heart attack at work this has been a real wake-up call for him i should have come to doctors yeah typical man you put it all off generally i think men are really poor at going to the doctor men don't like to talk about their health i think women are much better at going to seek help and advice carly checks his heart rate and blood pressure they need to rule out a serious heart condition what's your diet like i don't need health fifa i've had like you want him a burger today does anything make it worse apart from pressing when i'm pulling the cage when you pull in the cage that way he pulls on it yeah feels like i'm pulling something do you feel that you get like a pain going up there with it as well yeah okay as well as his unhealthy diet carl also tells them he's a regular smoker have you got um a wheelchair that we can borrow just to get a gentleman out to the ambulance no if you i mean if you've got the chest pain i'd prefer you not especially with the shortness of breath as well i'd prefer not to get you up and moving chest pain can be a number of things it can be the muscles in the chest wall it could be the heart i think chest pain is a relatively scary thing to to have while you're standing up are you able to whip all your tops off for me yeah please if you don't mind then bark your bum on there [Music] that's it feet up so point to where this pain has been here right there can i get you to sit forward for me just so sorry i'm just going to examine your back first of all and i'm just going to have a little listen all i want you to do nice and normal breathing for me all right you don't have to make any extra effort or anything lovely okay and sit back for me just gonna have a little feel over this part of your chest and i'll focus on the area that you've got the the pain as well let me just i'm all i'm having a good feel around all of it just there a little bit there was a little twinge then quickly yeah that's that's there yeah that's what it is i can feel it there is it's right on the muscle sorry it's alright it is right there isn't the signs are pointing towards muscular pain but they need to do one final heart trace to double check there's nothing more sinister going on carly i'm going to pop some extras some some of these sticky dots on unfortunately they're pre-shaved you trick your chest for me any heart problems in the family carl no no no apart from the inverted t waving three that's absolutely fine if i take that off we'll get you to pop your top back on if you want all right because now i've examined you the ecg isn't showing anything alarming but something's clearly causing the pain karl needs to be checked out at a and e okay carly whenever you're ready okay leaving scene 15 22. thank you [Music] you know your shortness of breath yeah that you've been having is that worse than what it would be normally because obviously you're a smoker yeah today's been like the one the worst out because i just they all fed it hard but i was trying to do my job i found it hard to catch my breath i feel fine do you know what i mean yeah it's fine filled with light headed you're all right yeah he's felt a bit lost again in a big breath is that all yeah how many do you smoke a day roughly oh 12 15 20. rowley's or normal air balls really sounds like depends yeah i should give up earlier how long you been it's smoking years whatever [Music] 1552 at hospital thank you can i just put those on there is that all right thank you that's everything isn't it yeah that's fine okay couple of months now down there ah damn carl will now have a series of tests to get to the bottom of what's causing his chest pain um not while i'm driving thank you oh yes that's the political answer both hands firmly on the steering wheel at all times apart from watching i'm having to resist putting my feet up on the dash [Music] mike arrowsmith and gas clark are starting to feel the effects of a very long night shift oh good you know when you blink your eyes and it seems so nice to blink two minutes to the shift we've been pulling onto station the gates have been opening you've been given a job it's turnaround it's go if that job's going to take an hour it's going to take an hour and a half it doesn't matter you've got to go in the final hour of this morning's shift they get a call to a woman struggling to breathe red two clear left all clear clear shortness of breath warm to the top chest upper backpack features a cardiac apparently here we're going to go and say gazz and mike arrive on scene their body cameras capture everything first hand she's in the bathroom she's just been up because she's been sick right but she's been went to bed early last night feeling a little bit sick but i woke up this morning couldn't get her breath she's had her inhaler hello my love and what halfway but she's yeah well you can see the steak dog [ __ ] let's have a sit down where are you going to sit down where do you want to sit down i think i should just sit on the side of the bed denise and graham have been married for 53 years no you're not cold no you're not cold if you've got a bit of a temperature it makes you shake as well okay yeah how many puffs have you sold you tomorrow did you have about four or four just just pop your hands on your left right with the soul beauty smell if you have quite a few that can also give you the tremors and a bit of a shake although denise is worried by her shaking it's her breathing problems that are more of a concern to gas my fingers pins and needles yes cramping yes yeah cause you're breathing too fast what do you normally suffer from asthma no um copd mild mild ceo copd or chronic obstructive pulmonary disease leaves the airways inflamed and the air sacs in the lungs damaged the patient often has a tight chest and can't get their breath 80 percent so i've got 20 lung plus today right is that 20 left or 20 gone what's the difference are you working on 80 of your lungs working fine yes yes right sorry the other way is not very good yeah 20 is not very good it's not very good but you see when i asked you that question you suddenly paused and what the hell are you on about you stopped shaking i did yes sir you did when someone goes wrong especially with our breathing we cannot help but panic adrenaline is released into the body yeah upping your pulse rate and that automatically helps your breathing rate perfectly normal okay so you're calming down nicely in there right we'll take that she was fighting not to panic i was yeah she is a fighter which worried me with denise calming down gaz needs to do more checks to see what's making her feel so unwell right 39.7 oh that means you've got a radiant temperature that's why you're shaking when we get a high temperature we actually shake yeah but because when we're kids we know when we go out playing snowballs and we get cold and we start shivering your body's now shivering and your mind's going oh i must be cold yeah it isn't it's actually toasty it's a what's disgusting very toasty an infection normally yeah well i've had a an abdominal no what have i had graham i'm getting completely off for three weeks i've been on antibiotics you've had a tooth infection and tooth infection yeah which i've still got still going on at the moment her gp has prescribed antibiotics but they haven't gotten rid of it [Music] hmm did you bring that up just a bit not much not much right we'll get you a little drink we'll get you to coco the paracetamol will artificially lower your body temperature down which will make you feel better which will stop you shaking yes okay like you've suddenly stopped shaking in there every time i told you it must be the constant drone on my voice it's sending your body to sleep again it does to me all night if you want to have a little sip wet your whistle first although you're calming down now your breathing's absolutely fine we do know about the higher temperature which is going to be because of your infection that you know you've got going on that's perfectly understandable we need to make sure that the paracetamol that you've taken just starts to work starts lowering your body down and you're nice and stable so ideally wise if we take a little walk downstairs we can do some paperwork and what's your name sir graham right graham can put the kettle on then can't you oh yes yeah in the time it takes gazz and mike to do their paperwork they can already see the effects of the paracetamol on the knees you know got plenty of colour back you've stopped shaking and i'm sure you feel a bit better then yes yes we can take you to the hospital if you want to no no i don't no i don't blame you if i thought you needed to go i wouldn't give you the choice no problem if i could trouble you for a signature that's if you use the sharp end of the pen it tends to work better yeah thank you no problem at all very well yeah thank you very much before you go no we finished quarter of an hour ago it's 7 15 in the morning and time for [Music] bed [Music] 999 mode activated it's 9 pm and steve geno and neil weaver are on duty until the early hours they're responding to a call from a gp who's wrong 999 on behalf of a very young patient [Music] female difficulty breathing and calls being made for a doctor patient has got asthma [Music] for a baby who's struggling to breathe it's blues and twos lights and the siren [Music] stop there you go [Applause] [Music] that's the first time i've ever taken a tram despite the traffic they arrive at the house in just six minutes and the baby's mum is waiting for them yeah right what's been going on with it the last four days she's been really really barking like took her to poppy has a low immune system so she's more susceptible to germs that other children might just fight off she's been really struggling [Music] for now she's just really really she's been really sort of barking with it as well the doctor did give her a prescription but poppy's mum hasn't been able to find a pharmacist who stocks it i have fun literally every homie can we we already do a few checks on her in that is that okay have she been in herself miserable apart from that apart from now she seems all right poppy hello can i put this on your chest and have a little listen would you reckon yeah are you wanted you noticed that she's had any sort of like a threat that's been sort of like sucked in at all i've just been doing sitting there no ribs have been yeah okay if you put it on your back yeah the problem with children is that they can sort of like cliff edge even though they look all right they sort of compensate and compensate and compensate and all of a sudden they sort of drop and become very ill quite eating quickly drinking as normal not normal because i throw it really sore through coffee so she's finding it hard to swallow can we put this in your ear is that okay [Music] well done well done okay 37.5 so she's got a little bit of a temperature just to check her blood sugar levels because with kids they can drop quite quickly they give poppy calpol to bring down her temperature then prepare a special seat for her in the ambulance [Music] tell me to keep you safe there you go do this i use this who's that he needs to go in the wash i'm gonna try and get it off here to put it in the wash don't put the washer wish you when it comes to using entirely fine she comes straight obviously when i've got the thing get ready for that sure she just takes it lovely [Music] like a model kingdom in you really with children the main battle is getting to sort of trust you in so if you can interact with them if you can sort of like make game games and things with them then it's after battle really [Music] oh yeah leave me alone where did you go where's she gone where's poppy gone oh there she is oh she's gone again right we're here now we'll see if peds is open mate oh god there's two trucks here already ready they take poppy straight into the children's a e department where she'll have to have an x-ray on her chest to see what's making her consistently ill [Music] [Music] [Music] it's lunchtime and they're racing to a 79 year old diabetic she's unconscious in a cafe what's the perfect place to be in if you're gonna have a hike a cafe will have one of those yeah penny needs a bacon sandwich to bring the sugars up and we're feeling a bit busy oh glucagonal glucose tough call they arrive seven minutes after getting the call hello val is just starting to come round how you feeling [Music] she take insulin tablets or yeah sharp scratch they immediately test her blood sugar but it's normal showing her diabetes is not to blame foul can we get your ammo to this jumper [Music] you got any pain anywhere headaches dizziness you just feel tired it's a dizziness maybe a bitter dizziness a couple of bumps as we go out val there's the first they've tested val's blood pressure and discovered its low they move her into the ambulance venting episodes are always worrying in the elderly the main worries uh are cardiac um we have to roll that out and be 100 sure on that um before we decide a plan of action for a potion so what were you doing in there were you having something to eat i just had um a little drink yeah yeah did you felt unwell today not really did you get hot you might have been yeah yeah yeah did you pass out lose consciousness no no no no just an older lady what you yeah but you're very clammy as well so something's happened so let me try and get to the bottom of it when my colleague comes back see what we can do so how well controlled is your diabetes [Music] [Music] should we ask your husband when he's had his copper better yeah otherwise you're not going to get home are you no no how old are you what day and month were you born and what day [Music] you got to know this otherwise when are the president's gonna going to have any come i need if your husband knows your birthday is a man with a plan bringing his brew with him dan's still no closer to working out what's [Music] which is scrambled egg beans tomatoes hash brown and then and then did she just go a bit dizzy but where's there no i just spouted i've gone i couldn't like rub okay i didn't that well you wouldn't know so she's got diabetes she diagnosed with any dementia yeah yeah just mind your arm on this side i'm just going to drop this side and you can swing your legs round sit yourself up see how you feel when you sat where's the legs on the floor bless you val's temperature and heart rate are fine and her blood pressure has returned to normal so they want to see if it drops again when she stands how do you finish the door all right okay it's normal for her to stand like this okay she has val took a while to to get back to normal we'd ruled a majority of things out with the the observations we could do but in the end everything was fine well i mean it's a lot of trouble for the hospital because it's never trouble for the hospital okay take it home and then we will find the doctor yeah all right straight home let him look after you can you look after him don't stop now then there you go shane helps val to the car just to check she's okay walking for the first time look at them sweets you've got in there they're sweets no wonder you're suggesting so are you it's nice don't you [Music] a lot of people focus on these horrible medical conditions your cancer and you know strokes heart attacks however dementia i think is is brutal because you lose that person piece by piece to the point where they're incomplete now and you've lost them mentally they're not there but inside the person is still in there somewhere they just can't access it you've got all these memories of your life trying to get they can't remember they don't even know who you are do they just like that must be awful for the family carl who had chest pain and struggle to breathe at work had a series of tests at hospital luckily it turned out to be a badly pulled muscle he was given a course of painkillers and was back at work after two days poppy was given steroids to treat the croup and discharged despite tracking down the gp's prescribed medication her low immune system continues to cause problems she's since suffered tonsillitis [Music] denise went to the gp two days later in addition to her infected tooth they discovered she also had a urine infection she's on antibacterial tablets and her doctor is doing further tests [Music] paul sees a dedicated nurse for therapy sessions and takes anti-anxiety medication he's being reassessed by his local mental health hospital to see if he needs further treatment he hasn't been back to work should we go yeah [Music] foreign
