hello hi oh God we go don't worry it's okay let go of me let go we're back on shift inside the ambulance oh got job you need me to get in the back you might have to stop with more medical emergencies come on okay ooh that's a good one and more cameras what that that Tyler is a little camera we're taking you back to the heart of the action just lie down for me we're from the ambulance okay I don't want this face there are some new faces I'm happy to be your crew mate I feel like we've bonded and some old friends I be driving you is it the fact that I'm faster and smoother than you your driving's terrible how can I help body mounted C record every moment nice deep breaths go open up Smash in to show you what goes on behind closed doors we'll get you right all right you'll be on the front line with the ambulance Crews from West Midland's Ambulance Service I don't need to be a doctor I just need to know what I can do to help as they deal with 3,000 emergency calls each day we see so many people you get told that NY seword there so many people can fight it now step inside the ambulance you take care mate all right mate thank [Music] you have you even got a Christmas tree up yes have you got a star on the top of it yeah it's taky as well was such a bar humbug this year I got no decorations up I've got no room for a tree youve got no room for a tree I live in the world's smallest shoe box there's always room for a tree there not to be a big tree B need have a tree have you got a Christmas tree up we have we went out yesterday and bought one have you yep we bought a real one for the first time ever is it it is massive it's pretty much taller than me to be fair what's your color theme going to be I'm thinking ice cave that's Wy and then uh you w't think it was two Lads living in the house would you to be [Music] [Music] fair West Midland's ambulance service is on call to an area of more than 5 million million people do you get the keys no it's the week before Christmas and another busy shift for paramedic Lee Farley and student paramedic Nicole mclintoch 999 Road activated a new job is just [Music] in we're going to a a male who's uh under the age of five is having a fit as the patient is a sick child the call is immediately classed as category one an ambulance crew's highest priority go right here right here yeah uh it's 12 month old it's had a previous febal convulsion he had vaccinations today it takes them just 7 minutes to get to the house the baby's worried dad is waiting at the door he's still on the phone to the ambulance service hello great thanks for your help thank you where we going thank you hello where that hello can I put the main light on yes of course yes it's a little Joseph is it yeah okay let's have a look at Joseph is he fully awake now um not really Joseph not reacting one-year-old Joseph was born nine weeks premature and very underweight at just2 11 oz is that his jobs today yeah he okay injection he's just had his routine menitis B jab normally fit him well yes he's had um two seizures before but that's when he had a temperature as well okay any temperature at the moment he can Spike his temperature after having this jab but the last two he came out really quick didn't he like C yeah and he threw up the inque and then he was like H the previous ones how old was he then it that was about um 3 weeks ago did he oh okay so still quite recent yeah did he go really stiff or was he shaking giving him a bottle and all a sudden he just stopped and you could tell we almost expected it t I sort of like rolled did they yeah okay well we're going to check him over I think that's an interesting thing this one piece will make 52 layers watch on mobile devices or the big screen all for free no subscription required download V now Nicola called 999 when she became worried that Joseph's convulsions were going on too long we just told didn't we my hand they information if it get over five minutes you need to call and and what was it in the end minutes that 8 minutes in total okay no problem so febal convulsion is when a child would [ __ ] their temperature and it's the body's way of coping with it if they go on for too long anything more than a couple of minutes the breathing pattern changes the heart rate changes uh it can reduce um the oxygen to the brain and we need to act quite rapidly just to to improve things with that child is any in the cowp or anything six yeah any marks rashes at all mon yeah we we just noticed he's got like a bit of a rash around here he's not suffers with like problems with his breathing since being born he had um brontis um when he was about 8 weeks old and then he's had a chest infection as well recently and that's why he had the um seizures okay what we're looking at here is just getting a heart rate and oxygen levels just because he's had a seizure whil having a seizure they're not breathing as well as they should so we just want to make sure that's not been impaired because you've had this febal convulsion would you be happy if we pop him up to the hospital getting checked over by a doctor yeah but then Lee notices something even more worrying normally a little bit M on the back of hands as we as we legs are the same okay he's coming across the chest as well now all right um can we uh pop a traa mask on yeah just so give me some give me some supplementary oxygen just try and perk him up a little bit mottling can be a symptom of menitis o oh let meand a connector there you go there for Joseph's big sister Violet whose fall the whole experience is very frightening we're making better he's just having a bit of oxygen to make me feel a bit better okay oh that's okay there we go are you helping to look after Joseph yeah sister Santa Claus will see that we've got a big telephone in the ambulance and we can talk direct to Santa so should we tell him that you've been a good girl yeah luckily the Oxygen's improved Joseph's modling meaning it's probably not menitis but it's still unclear what's caused the convulsions m going get settled on the ambulance okay I'll Follow You D all right okay bye-bye thank you no worries okay okay Mom you want to sit yourself there pop your seat belt [Music] on uh Nicole yeah I'm happy to uh proceed okay it's the second baby we've had today yeah we had a 5mth old earlier so this is just um so we can be assessed by the Pediatric nurses and doctors uh a little bit of reassurance for yourself but so just getting checked over just cuz he's had the previous they just grow out of it as the years go on and the body KS through temperature a little bit better they do and they stop scaring their mommy and daddies yes FAL convulsions can be really scary uh for anybody especially parents I've been through it myself I've seen my child have a a Fe and although I'm trying medically uh it's still of concern his breing rates coming down so that's that's good that's good yeah oh no we're here now what are we going to do we'll take him in m and hopefully go straight around to the Pediatric area all right you're all right yeah yeah yeah it's about time to do some [Music] exercise it's going cold outside so I brought my hat yeah more that keep your big head warm yeah there you go hey Merry Christmas look at ball flopping all over the place he was cute wasn't he yeah it's another baby today just wasn't wor all I don't like the way his hands were mled so oxygen it worked do it and it worked well yeah a well he safe now yeah bless him he looked at good we um Ginger touching his hair he had yeah was Ginger we have to stick together [Music] what's the worst Christmas present you've ever had 12-hour shift on Christmas Day the worst Christmas present never got was a joke book from my N I also bought one for my brother my dad I with three uncles so the Christmas dinner T was a bit like why did chicken cross the road p I don't think I've ever had a bad present there some confusing presents I've had that you get them when you think why in the hell I been given this my brother once bought me a set of nipple tassel and like a male thong it's like a joke present subsequently they were never worn yeah I believe you [Music] in Dudley on a late Frosty December afternoon paramedics Darren Westwood and Steph Jay have just been alerted to a new patient who needs their help c three we're going to a 65y old male with an injury falling out of bed with a shoulder injury in Tipton this a bit icy you have reached your destination your collarbone a little bit what we're going to do he's going to pop it in a support sling take the weight of the arm and then tie it up here do you want some gas in air some ento some pain relief I don't need something yeah go grab the gas [Music] when I'm I'm being took toal yeah we're going to take it to hospital mate because it's going to need to be x-rayed all right all right just the more you tense up the worse it's going to be all right try your best to relax as much as you can this sand can move move this hand out the way have you had gas in a before need to hold so yeah hold this with your good hand pop it pop the end into your mouth say relax it pop the end into your mouth and take nice long socks on the tube nice big deep breaths bite onto that bit okay keep going on the gas it help take the pain away strong deep breaths I want to to make a whooing sound it's better Steven has a brother and sister who live locally and who he sees regularly have your cats been fed tonight the water be is he going to pop in and feed them or do we need to feed them before he before we go he be as long as I'm in this room yeah then they've got food and water cuz see they've got water where's the [Music] food brother be down yeah okay before they head to hospital he wants to collect a few things to bring with him say sorry you don't mean to fall out did you we'll have a look in there let's go and find your phone Steven used to work in a Foundry making parts for train engines see down there yeah just off the side it's a very high bed this isn't it yeah you have to take a running jump to climb into it in the belt that's your belt right take that out and where's your phone there's your phone with step's history of falling out of bed he could have quite easily broke his collone or dislocated it which I have suffered before and I know how painful it is as I say hopefully you'll be done and dusted out for for tonight with some P relief and then hopefully it's pick bar Christmas not before I hope so hopefully mate where's your brother lived in just a little walk on the canal and is he okay to walk around tonight for them cats he' be all right Miss cats mind still the company aren't they for you yeah a way you say you're going to run your sisters for Christmas yeah well I'm glad you've got somewhere to go for Christmas anyway I won want you to be on your own I think some of us don't realize how lucky we am to have a a family sometimes to go to oh yeah there's lots of people out there lonely really aren't they I see some like even the last few days with this on the floor yeah there some about I think over it this is it and Tumble over where you they tumble over but you you're fairly resilient mate I don't think you've done that much damage so don't get upset I think you'll be fine if he was any different we'll be going on blue lights wouldn't we mate and we ain't we're having a little pole into Hospital they'll go and fix you he'll have some pain relief and then once that's done you'll be back to your Merry s you be had a good on shopping and everything I think we're here we here B all right no running rsh I got you oh trust me now I've got there we are my lovely Steven will be taken into a and e where they can manage his pain and x-ray his collar bone [Music] Which Way left got left running a m away from it's December and Christmas isn't far off ambulance crew mates Craig olop and Tony Williams are working a day shift and their next emergency has just come through we're literally on top of this [Music] drug arriving at the patient's home they're met by concerned mom Mandy hello sweet all right she was worried that her 25 year-old son John who has Down syndrome seemed very unwell and he was concerned he couldn't breathe easily so what what's going on then what's happened don't don't worry you're right we're not going to take you anywhere it's unnecessary can I ever sit by you is that okay I like your Jammy bottoms it's an exacerbation of it today thanks hello I'm Tony you okay no we just want to have a little look at you don't be upset don't be upset it's you'll get me crying if you're crying you just want to check your okay joh how does your breathing feel at the moment fine it feels all right yeah are you going to tell us you feel fine John had Down syndrome and other learning disabilities as well he he was really upset he was he was distraught I felt for him so so much this family had two strangers walking into their home in their full uniform with all our kit and all our bags it can't be a very nice experience especially at Christmas time you got to be honest with us all right John if there's anything that's that's different to normal you got to tell us all right John has several underlying medical conditions and has recently had a nasty chest infection they need to work out what could be causing the problems today and whether John needs to go to hospital what we'll do we'll plug you into machine and I'm might to have a listen to your chest in a minute I'm that's fine can I just have a listen to your breathing just for a minute yeah just see if your chest nice and clear can can we can we take your hoodie off is that okay have this on your arm sweetheart bit cold I'm afraid oh you are absolutely cooking so as soon as I touched John Skin he was boiling I knew he was running a temperature and that was a serious concern for us because a high temperature is a sign of an infection John really didn't want to go to hospital but it was really important that he did get medical attention as soon as possible has you had any paraset or anything not today now no right it's got a bit of a temperature yeah so i' probably think's got something going on whether it's just a continuation his chest infection I don't know have you got any pain at all John any aches and pains no no are we right to turn the fire off is that all right we need to try and cool him down a little bit he temperature 375 it's not it's not ridiculously high but it's high enough to kind of be concerned so really I'd like with his history to to take him but I don't think he's going to want to go is he but yeah gock over come down I'm coming down with you the problem is it's only going to get worse isn't it if we don't sort it out now are you going to come with any your mom your nan right so what if your nan comes with you and stays with you until you come back home mom I'll come down to you in a minute I'll come back home you you will come you will come back give you a quick check up and off please yeah yeah we can take it off they'll check you over all right I'll check you over everything is going to be absolutely fine they'll check you over we ain't going to do anything we do anything to hurt you all right hey John we got to get you better for Christmas haven't we what do you want for Christmas what do you want for Christmas laptop a new laptop new laptop oh so we can get you better you can you can have a go on your that's up a Christmas can't you and you'll be r as rain the sooner we go the sooner we can get you back can't we yeah back you'll come back I promise you'll come back all right you'll come back yeah yeah we got a deal yes good man with John feeling calmer up we go there matey Craig and Tony help him onto the ambulance should we pop this on you just try and stop got that shivering a little bit eh pop your arm in this one John okay right all right John there's nothing to be worried about all right nothing at all John we're going to give you a little oxygen mask all right just to get your oxygen levels a little bit higher than what they are just the how are you feeling how's that breathing feeling now if you telling me the truth are you feeling a bit poorly no no is that the truth you promise you're not going to tell me fibs are you um we'll get off mate yeah cool right John Craig's going to take good care of you and I'll see you up there okay for the short ride to hospital Craig wants to keep John's mind on happier things so you going to get some games for your laptop then at Christmas what games do you like playing what's that oh bit WWE yeah who's your favorite [Music] wrestler Triple H Triple H yeah I I used to play it on the PlayStation one when it was WWF I used the play on I was rubbish I was so bad how you are you good yeah you probably beat me [Music] then you that's for into any time a few minutes later Tony pulls up at hospital how we doing John you okay it's all right now fabulous is he looked after you yeah I'd be having work with him if he didn't you me and he looked after you that's good okay then buddy are we ready pop your arm in that's it a few bumps John John will go to a andd for further checks but hopefully his stay will be a brief one right then let's get you in the warm freezing ENT it [Music] oh we were so upset I think he is a bit distraught really the fact that he thought he was going to hospital I mean obviously he had to go um in the end but I think we reassured him a bit that there was a oh we properly talk to you didn't he he did did you see him hug me it was a proper manly hug yeah yeah it a good job that was yeah it was bless him though he's a nice guy and I hope he just get sorted oh [Music] [Music] well I was a vegetable well vegetable would I be I'd be a potato lots of Versatility to that vegetable I think I'm most like a marrow a marrow marrow I'm classically British and with a bit of love and care and attention I just keep growing bigger that's hilarious [Music] [Music] [Music] in Dudley ambulance crew Steph J and Darren Westwood are focusing on the festive period what about Christmas movie is MW after you've eaten your dinner what did you settle down to watch my I nor before asleep I said I get you know you get the food sleep yeah get you get so stuffed you fall asleep the crew have received a call to a 40-year-old man who's collapsed he has severe epilepsy and history of violent seizures [Music] you after 7 Minutes the crew arrive hello and find Carl unconscious luckily his friend Tracy was visiting and called 999 so what happened then my love he literally just got out the sh out come out he'd already eat the dick what what actually happened when was he shaking or anything else like can you hear me sweet you open your eyes my lovely he landed on this okay could be dragging him out the way here mate cuz he's going to end going down the stairs yeah I do me did he shake at all yeah he shaked arms and legs yeah yeah so it sounds like a full tonic clonic the type of seizure that Cole had was what we would deemed to be a grand male type seizure or tonic clonic so is actually fitting with all limbs Shaking All L back we need to establish the length of time we'd been seizing for anything longer than 5 minutes could lead to brain damage how long did this shaking last for did you know couple of minutes yeah okay have you seen him have a foot before how quickly does he normally come around afterwards seconds normally in and out yeah hello mate open your eyes for us Carl come on Carl wake up for me come on Carl wake up for me Carl's been unconscious for over 15 minutes it's the latest of several seizures in the last few days the longer a patient is out the more dangerous it can be Cary po a in your hands okay I Know Carl you're right on the edge of the stairs don't move anymore [Music] finally Carl is showing signs of coming round hello hello mate how we doing Carl how you feeling how you doing mate all right don't panic okay you're better a bit of the fit better fit my lovely you right to the top of the stairs be careful does it hurt at all it you got a big headache did did you know what's happened no you've had a bit of a a fit a had another fit mate oh no car what's hurting your ribs where abouts your ribs hurt my lovely any pain up here at all no that hurts okay take a deep breath again you on you on the stairs danger just sit up m a little sit up for me behind you we go that's it pull against us perfect much better how we feeling Carl do your head hurts bit of a facial injury here on your cheek this side this side your teeth are not hurting or anything else like that your teeth are in line can you taste blood at all have you bit your tongue feel no teeth all feel normal the crew want to get Carl to hospital and his inju is treated but they need to go carefully right nice and slowly down these stairs hold on come on you just get your G disorientation and confusion from seizures can last hours afterwards right pop yourself on the bed my lovely swing your legs up [Music] how we feeling now a bit more with it yeah back to normal or not I'm or your version of normal I'm do bit dopy bit Dopey still okay happy St yeah when you are mate my B my um when you fell you went face forward and hit your ribs quite hard from what you go what your partner said and that's why it's quite painful so just need to get those X-ray and make sure they're not broken okay then while in there we we can get that face looked out in your head so if you feel if you feel you're going to have another seizure just let me know all right yeah and you had four seizures yesterday [Music] good now at hospital doctors can treat Carl's injuries and get him the medication he needs [Music] [Music] ambulance crewm mates Ash Bolas and Stuart punter are on their way to a man whose chest pains and high temperature are making him feel very poorly D sure I'm coming down with something and it seems he's not the only one [Music] that doesn't sound good I'm I'm sure I've got something coming man my way I don't think it's as bad as man BL I think bit of a sore throat their patient is 57-year-old musician Steven he was urged to call the emergency services by his friend Katrina after he became unwell and had trouble breathing okay take a seat what's been happening tell us a story right i' got CPD okay yesterday I started cough cuz I got persistent cough any cu smoking in CP it's like somebody's got they every time a cough they're just doing digging in yeah and comes around here and CH whatever you want to call it around here and I've G like I can't get warm at all yeah and how long's this been going on for this started yesterday so this started yesterday we got really bad today Steven has has a lung condition which is often seen in smokers continued smoking can exacerbate the symptoms have you taken anything for that pain no okay any pins and needles or anything like that any jaw ache or nothing like that any vomiting at all when I'm coughing it's making me feel a bit noisy cuz I'm you coughing and heing excuse me Steven made us aware very quickly that he had something called COPD this is a life-threatening condition if left untreated we need to assess somebody rapidly and ensure that they're getting sufficient oxygen into their bloodstream so where abouts is this P if you just point to it it's it's kind of just there roundabout here all right just there press it open I'll tell you about there okay so it's just that constant and just do your temperature see how how we doing with that even more painful when I'll cough and right around the ribs and 38.5 that high it is High um it is higher than normal it should be between 36.4 to 37.5 um anything above that is sort of indicating some sort of either infection going on Steven had a temperature and also he was showing signs of a chest infection when a patient has an infection with COPD um if left untreated um the patient can deteriorate uh that's why we need to establish the right course of action fairly well know right just going to have a quick listen to you what's that sorry oh yeah 38.4 I think it was so it is up a bit heart rate is up a little bit okay take a big deep breath in give I know sorry okay let have a listen around the back as well all right you're definitely cooking a't you absolutely freezing you've got a little bit of a crackle on the right side yeah that's very dirty yeah so potentially is that you've got a chest infection yeah um with your temperature being elevated um your heart rate's a little bit quicker than normal but not massively Ang all right um I mean your oxygen levels are fine that's not a problem um it's just obviously the fact that you're go in hot and cold which is fever like symptoms um obviously you're coughing it's painful strong suspicion that it could be um a chest infection all right are you still in pain now yeah have you had any parasal in the last four hours no but she's just brought me some she just brought some ah she's uh looking after you while she yeah we used to we did each other we kids there's a photo on there behind you no way that's not you two is it yeah no where where no she's about 14 and I was about 17 something like wow no what year was that that's got to be 70 something is it 75 I 76 wow Steven's breathing may be better but the crew need to make sure he's not going to suffer a repeat attack if you've got a chest infection you're going to need antibiotics um obviously going to need chest x-ray blood test to to see exactly where the infection is and look at your inflammatory markers obviously we can't do that here the obviously because it's a weekend the only options are is either A&E or Urgent Care Center Our advice is obviously see a doctor as soon as possible it's up to you what you'd like to do whether you want to come with us now up to the AE or urgent care what what I'll do is I'll pop into um the walking for me tomorrow the sooner you get this treated the sooner you'll start feeling better cuz with COPD things can spiral Steven opts to take himself to the clinic the next day so Stewart and Ash are free to go all right lovely take care both nice to meet you don't let that that was an interesting case that we've just been on 9 certainly different well I think he'll go in the morning she got a strong feeling that he just didn't want to go out this evening yeah I understand why you don't want to go there on side Saturday night hospitals everyone presumes they're full of drunks and everyone lying about on beds vomiting yeah which is true to a certain extent [Music] [Music] yeah baby Joseph had no lasting ill effects from his convulsion a pediatric consultant concluded an ear infection had caused the high temperature that triggered it a dose of antibiotics later he was able to enjoy his first Christmas with his big sister Steven escaped the fall from his High bed without major injury his collarbone wasn't broken just badly strained and after a few days rest at home with his cats he was soon back to [Music] normal thanks to the paramedics wrestling f John faced his fear of hospitals with concerns around an underlying heart condition he was kept in and monitored overnight he was discharged the next day to spend an uninterrupted Christmas with his family Carl had had a severe epileptic seizure after several hours in hospital to make sure he was stable he was discharged to continue managing his epilepsy with three different medications to keep the more serious seizures at Bay [Music] and Steven went to a Walkin Center the next day he had a course of antibiotics for a chest infection and was soon feeling much better really I'm massively done in for the only the third or fourth time in 4 years I generally cannot wait to get home and get to bed [Music]
